If each shirt cost $18.00 and has a one time fee of $63 and there's 2 shirts how much is that

I've had some shirts tailor made.  They charges a on=time base fee (just for taking my measurements), then a per-shirt fee for however many I wanted made (once the pattern is made, each shirt is pretty easy to make).
 
This also happened when I asked the print shop to make a bunch of pamphlets.  There was a one time set-up fee and then a per 1000 copies fee.
 
Can you think of other examples?

Any example involving design of some 'thing' and then production of said 'thing' in repetition will follow a similar pattern when we need to pay a one-time fee to a designer and then a repetitive cost to a manufacturer...possibly design and manufacturing is handled by one in-house organization or person who bills us the collective sum for both tasks...as was the case in this problem.
