Point slope form is y − y1 = m(x − x1)
We were given (x1,y1) = (3,5) and we know the slope is m = 1/4
Plugging everything in gives
y − 5 = (1/4)(x − 3), which is the 4th answer choice assuming that there is a typo and you meant 1/4 instead of 14.
