center at (0,1) means x2 and (y-1)2 on top.
vertices should be at (-2,1) and (-2,1), since they should be left and right of the center! i'll assume that was a typo. with 2 units from center to either vertex, a = 2 and therefore a2 = 4.
minor axis points at (0,2) and (0,0) mean they are both 1 unit from the center at (0,1), so b = 1, b2 = 1
therefore:
x2/4 + (y-1)2/1 = 1
