First you would need to find the slope: m = (y2-y1)/(x2-x1) = (6-1)/12-4) = 5/8
Now remember the point-slope equation looks like: (y-y1) = m * (x-x1) where x1 and y1 are x- and y-coordinates of a point given to you.
If you use the first point (4,1), the equation would be: y - 1 = (5/8)* (x-4)
If you use the second point (12,6), the equation would be: y - 6 = (5/8)* (x-12)
