First find the slope by finding the difference of y coordinates over the difference of x coordinates. In this case the y coordinates are 1 and -3, and the x coordinates are 4 and 8.
So the slope (m) would be 1--3

Then you plug this into slope intercept form (y=mx+b) along with (4,1) as your x and y. You can then solve for b (y-intercept).
Now that you know m and b, you can write the equation in slope intercept form.
