Multi-charged ions are often insoluble when they are paired with each other; this is because having lots of electric charges on something that small makes it possible for them to attract each other strong enough that they will squeeze water out from between them the way that water does to oil. That is why the PO43- has to be paired with Na+ and Mg2+ has to be paired with NO3- in this question so they will actually dissolve on the reactant side. When Mg2+ and PO43- are paired together, their multiple charges will bind them to each other so they will come out of solution and form a solid.
