The point-slope form of a line can be written as (y - y1) = m(x - x1) where m is the slope and a point on the line is (x1, y1) so this line can be written as (y - (-1)) = (-5)(x - 2)
I'm unsure how much your teacher would want you to simplify this, I would probably write it as y + 1 = -5(x - 2)
