Presumably you are thinking about a belt running around two wheels and you want to know the distance between the centers of the 2 wheels.
Since the belt is perpendicular to the radius at the point of tangency, one diagonal is sqrt(144+64)=sqrt(208)
Using this diagonal and the other radius, the distance between centers is sqrt(208-9)=sqrt(199).
