Alonzo is the head of his department. He wants to improve the amount of accidental falls among patients in recovery. Alonzo has a team meeting to discuss how many accidental falls the recovery room experiences in a year. He stresses the need to get those numbers down. He gives the team a goal of six months. What did Alonzo leave out that would allow his team to overcome this issue?

An odd question for the topic of "chemistry," but nonetheless ...
The most important thing that Alonzo left out was ... data! To even have a shot a reducing "... accidental falls the recovery room experiences in a year," the team needs to collect some meaningful data. For each instance the date, time of day, location, injury sustained (if any), staff involvement (if any), etc. needs to be compiled. Once some data is available a Pareto chart could be the beginning of some understanding of the probable root cause(s). Classifying groups of incidents with common characteristics can provide more insight to the problem and the beginning of mitigating potential cause(s).
