150 mg of an antibiotic to be given every 6 hours. Your child weighs 30 lb. Your research indicates that 20-40 mg/kg/day is the recommended dosage. Is this prescription safe for your child. If not, what would the maximum safe dosage be every 6 hours?

Ah, should have looked at this one first, as it has the weight of the child.  Let's start with that.  Actually, let's start with the safe dose, as it's given in "per day".  If we divide that by 4, since your dose is 4 times per day (6 hours), we get 5-10 mg/kg/dose

 

30 lbs * (1 kg/2.2 lbs) * (5mg/kg) = 68 mg/dose

30 lbs * (1 kg/2.2 lbs) * (10mg/kg)=136 mg/dose

 

So no, your dosage is too high.  The maximum safe dose would be the 136 mg for each 6 hour dose.
