Which, if any, churches have reunified with the Catholic Church?

Question

You hear about individual parishes (usually Episcopal) converting en masse to Catholicism, but has there ever been a precedent for an entire church converting to or reunifying with Catholicism?

None that I am aware of though there are discussions between Anglicans and Lutherans with the Vatican on this subject. Given the theological divides, my hopes are not high but those who are seeking that possible reunion are more liberal in their theology so perhaps it's possible.
