Look at the molar ratios in the chemical reaction equation. For every 3 moles of calcium carbonate there is 1 mole of calcium phosphate created. That means for every 0.4 moles of calcium carbonate, there would be 0.40/3 moles of calcium phosphate created. But since you only had a 95% efficiency, only 0.95(0.40/3) moles was created.
