 Not sure what to add.  I feel the patient over took the pills.  How many days ahead of the instructions is he?

It depends on what date the prescribed pills were obtained. 22 pills have been consumed--that's 11 days worth.  If prescription date was eleven days ago, everything is OK.  If the date was more recent (closer), then the rate of pill consumption per day has been exceeded.
