is there a proper way to email teachers about students you are tutoring?

What is the best way to ask a teacher for his/her input on a student that I am tutoring [I have permission from the parents to contact/communicate with the teacher(s)]?
 
Obviously, I do not want my email to offend the teacher. I want to reach out to her in order to gain information to assist with my instruction; to work as a team in the best interest of this child.  I want the teacher's opinion on any specific gaps she is noticing & what/if any skills/concepts that may need re-teaching for understanding and retention, as well as, the student's learning needs that she would like me to work on.  My hope is that through this collaboration, we will be able to empower this student to be and feel more successful in class this year and over the summer.
