The person weight 246/2.2 = 111.82 kg
40 mg x mg
-------- = --------
1 kg 111.82 kg
x = 40(111.82) = 4472.8 mg total received per day
Every 6 hours mean 4 times a day
So per dose patient receives 4472.8 / 4 = 1118.2 mg

I would change the original dose to lb
40mg/kg/day
1kg = 2.2lb
so 40mg/2.2lb/day
now 2.2 lb = 40mg per day
246lb multiply by 40mg and divide by 2.2 lb therefore, 246 x 40 = 9840mg and 9840/2.2 = 4472.7mg per day or
4472.7/4 = 1118.1mg per dose
