Upon exam she is clinically unstable and persistently febrile (T=39 deg, BP 70/40, heart rate 118). A sepsis protocol is started and you order IV fluids and broad spectrum antibiotics, including the glycopeptide antibiotic vancomycin. The patient weighs 58 kg, serum creatinine clearance is 0.6mg/dl, height 170cm.
