Flow rate is amount/time. = mg/hr
Things you know:
amount in solution: 60mg/12ml = 60 divided by 12 = 5 mg/ml
volume rate: 3ml/hr
Therefore:
5 mg/ml * 3ml/hr = 15 mg/hr (you can see how the ml in the top and bottom cancel our and you are left with mg/hr)
