I'm going to assume that your first point (o,k) is intended to be the number 0 and not the letter 'o'...
In slope intercept form (y = mx + c) m represents the slope of the line and c is the y-intercept. To determine the slope, m, just remember "rise over run". That is, given any 2 coordinate points (x,y), the slope is going to be the difference between the y values (rise) divided by the difference between the x values (run). For example,
m = (2k - k)/(h - 0) = k/h
Now to get the y-intercept, c, we just need to know that the y-intercept is the point on the line where x=0. Luckily we are told that the line passes through the point (x=0,y=k)! So, the y-intercept is
c = k
