It is just my personal opinion, but I don't think there is any struggle in the U.S. legal profession with regards to fiduciary concepts for any reason. The law is very clear that a fiduciary--legal professional or not--is required to perform duties to with utmost care. I am curious why you suggest otherwise.
Does the legal profession in the US struggle with fiduciary concepts because of its "underpinnings" of Common Law?
