The first step in creating an ERD is, not surprisingly, identifying the entities. The candidates seem to be applicant, application, school, recommendation and (odd tho it may be) bowling center.
Next is identifying the relationships and their cardinality (one-to-one, zero-to-many, many-to-many, etc.) Any many-to-many relationship should be resolved by an associative entity which reduces the many-to-many into 2 relationships that have a zero or 1 cardinality on at least one side. (There does not appear to ba any many-to-many relationships in this case.)
Finally, at least a few attributes of each entity should be identified - at a minimum, a candidate key (applicant id, school id, etc.)
