Write the table this way:
input x output y

Now, notice that input values of both 1 and 4 produce a value of 6 as output.
A function must have only one output value for a given input value, therefore:

For example: f(x) = x2 is a function even
though f(x) = f(-x)...
i.e. f(2) = f(-2) = 4 and so forth.
Every input has one and only one output.

I very much agree with Andrew. The words "one output is paired with two outputs" must be a typo and it totally confused me. Sorry, I'm not ready for "trick" questions yet this morning.
