This is a question (or prompt) asking for your opinions and thoughts. None of the tutors here can do that for you. What I can do is help you get started, and get organized.
If I remember correctly, Wormwood's "patient" is a Christian. So, it might help to consider if there is a difference between the guilt a non-Christian feels, and the guilt a Christian feels. It may help to consider the spiritual position or standing of the one feeling guilty. I suggest looking at Ephesians 2 for Paul's view of the change that takes place in a Christian's life. That could have an impact on how you look at this issue.
After you have your thoughts clear, work on a thesis statement that expresses your opinion clearly. Plan to organize your thoughts in a logical way ("good" guilt, "bad" guilt; feelings in children, teens, then adults; etc.). When you write your response (essay), begin with an introduction paragraph, and end with a concluding paragraph.
