The paper can be made into the cylinder in many orientations. Either the 4cm or the 11cm can be used as the height of the cylinder. As for the radius, how narrow and how wide determines the radius of the cylinder. Because of this, you need to specify your orientations and radius.

T.S.A = 2πr2 + 2πrh
where:
r = radius
h = height

My mistake. You can make a cylinder, you cannot find the T.S.A. - total surface area - without two bases.
