You must increase the weight for your patient due to the principle of progressive overload also called specific adaptation to imposed demands (SAID) principle. This principle explains that our bodies are able to adapt to demands placed on it and later requires progressively greater weight/stressors to continue building muscle. This explains why as you exercise consistently, the weights you lift feel easier and easier because your body adapts to that stress.
