If you are looking to create designs meant to be heat-pressed on shirts, then you should create your designs using Adobe Illustrator, since the designs there will be "vector based"/"smart images". If you are however planning to create shirts with complex real life sceneries on them, then Photoshop will be your best bet, however heat-pressed shirts of pixel based designs will require special machinery and the image will be in square or rectangular form.
