If you need an additional pound of concrete, that means you are adding +1 to c
w + 1 = (c+1) + 5
The slope is a 1:1 ratio, and thus you only need to add 1 gallon of water to get 1 additional pound of concrete. All the +5 says is that once w = 5, you have 0 pounds of concrete.
When w = 6, you have c =1 lb
w = 7, c =2 lb
w = 8, c = 3 lb... etc
