How do you perform a nursing assessment of the eyes?

Nurses use the PERRLA method: Pupils, Equal, Round, Reactive, Light, Accomodation. There are other types of assessments as well including Snellen Eye chart, ophthalmoscope, slit-lamp ocular to name a few.
