The net change in this case is just
f(3 + h) - f(3).
You have the function f(t). Evaluate it at t=3+h and at t=3, and take the difference.
The average rate of change is
[f(3 + h) - f(3)] / [(3 + h) - 3] = [f(3 + h) - f(3)] / h
