To find the scale ratio you would need to have both the "1 cm" and the "5 mm" in the same unit, then you could just use the numbers. For instance, if the map scale was 1 cm (map) to 10 cm (actual), then the scale would be 1 to 10.
In this case, you would need to convert the 5 mm into cm. There are 10 mm for every 1 cm so 5 mm = 0.5 cm. That means the scale ratio is 1 to 0.5
