You are given a function h(t), and you are told at what times t to evaluate h(t). Just substitute t = 0, 5, 15, etc. into h(t) and get the heights. There is no mystery to this.
By the way, the given h(t) suggests that the vertical acceleration is -2(16) ft/s2 = -32 ft/s2, which is unrealistic for an object traveling through water. Where did this problem come from? Anyway, that is a separate matter. You can only work the problem as it is stated.
