Your slope is going to be rise over run. So if you plot those points on a graph you will be able to count the rise (going vertically) and the run (going horizontally) from the lower point to the higher point.
If that is confusing then this formula is the same thing, m = (y2-y1)/(x2-x1)
m is your slope, and that goes into the second equation
To find the linear equation you use the formular, (y-y1)=m(x-x1)
