Hi Danielle,
To convert an equation from rectangular form to polar form simply replace x with r cos θ and y with r sin θ.
3x - y + 2 = 0 becomes 3(r cos θ) - (r sin θ) + 2 = 0 or 3r cos θ - r sin θ + 2 = 0
