Let A = Age.
Let R = Rate.
If A <= 5, R = 0.
If 5 < A <= 12, R = $10.
If 12 < A <= 18, R = $25.
If 18 < A, R = $35.
For a histogram, Age would be on the X-axis and Rate would be on the Y-axis. You could see the heights in the bins. You could also enter these into a table.
