If you plug in x=0 into your equation, the y-intercept will be (0, 16).  This is different from y-intercept (0, 8).  Also you chose values of x1 and y1 that are not consistent with a single point when you used them to find the equation.
 
You should either have
 
 
x1=0       and      y1=8                       
 
                  or
 
x1=-4         and          y2=0
