APA format for SOAP note from Nurse Practitioner

Question

How is a SOAP note written in APA format?

Need to write the Subjective portion except ROS.

I turned in my assignment.

My instructor stated that it will only be regraded after I redo it.

The thing is, I don't know how.

We've never been shown an example.

The cover page was the same APA format I've used all during graduate school, now, she says it's wrong.

Please help!

In short:

Subjective (S): Patient's feelings, experiences and concerns, in their own words (ie, Chief complaint)

1. ROS would go here, if needed

Objective (O): Observable and measurable data collected by the clinician

1. Labs, X-rays, etc would go here, if needed

Assessment (A): Clinician's professional judgment and diagnosis.

Plan (P): Treatment plan, follow-up, and next steps.
