The answer involves a messy bit of algebra...not much calculus.

Differentiate the equation term by term by term to get

2x+x(dy/dx)+y+2y(dy/dx)=0

Collect terms

dy/dx=-(2x+y)/(x+2y)

Now use implicit differentiation again using the quotient rule...and this gets messy.
