For this problem, you take the order (4 mg/m2 /day) and multiply it by the child's BSA (1.1). You get 4.4. Then, you need to take that answer, which is the appropriate amount of mg per day, and divide it by 4, since there are 4 divided doses, to figure out how many mg/dose. You get 1.1 mg/dose. To get that into ml/dose, you take 1.1 mg and multiply it by what is available 5 ml/0.6 mg. The mg's cancel out on the top and bottom, so you calculate 1.1 x 5 then divided by 0.6, and you get 9.166 ml. You can either round that to the tenth's place and get 9.2 ml or to the whole number and get 9 ml.
