H(x) is the product of "x" and "f(x)", so you will need the product rule.
H'(x) = first (x) * derive 2nd (f(x)) + 2nd (f(x)) * deriv 1st (x)
H'(x) = x [ f '(x) ] + f(x) (1)
So H'(4) = 4 [ f '(4) ] + f(4)
And you will need to look up f '(4) and f(4) in the table to complete this problem.
