It's a combination of learned skill and creativity. You need to be able to know the basics of a chord structure or progression to know what notes to use that would sound good! Not any note would sound good in a specific chord or progression. And the other aspect of a good melody comes with creativity, it's basically 'composing' and coming up with a combination of sounds and rhythm.
