Nursing Diagnoses are not written using the name of a medical disease. Nursing diagnoses are written regarding more focused issues that the nurse can care for and observe.
The nursing diagnosis also includes what the issue is related to and what it is evidenced by.
Example:
Medical Diagnosis: COPD
Nursing Diagnosis: Ineffective Airway Clearance related to increased production of secretions as evidenced by productive cough and dyspnea
