Intercepts/Slopes

A slope intercepts at (-4, 3), provide this a Slope/Intercept form equation as well as Standard form equation and a Tables of Values chart like this

Yes it is x-intercept -4 and y intercept 3.
I would like to know the Standard form (i.e x + 2y - 4 = 0) of the equation and in Slope/intercept form (i.e y = -1/2x + 2

Okay, great. The intercepts give you two points on the line, (-4,0) and (0,3). Find the slope using the slope formula.
 
m = (0-3)/(-4-0) = -3/-4 = 3/4
 
Next, you already know the y-intercept is b = 3. You can now get the slope-intercept form y = mx + b as
 
y = (3/4)x + 3.
 
The standard form can be found by rearranging this correctly.
 
(3/4)x - y = -3
 
3x - 4y = -12
 
3x - 4y + 12 = 0.
