You'll need to look at the instructions your teacher gave you but it is common to use the digits from your birthday as the coefficients of a polynomial. For instance:
Or, simplified, you get:
3x6 + 2x5 + 2x3 + 8
You can also mix up the operator between terms and instead of using all plus (+) signs, you can use a combination of plus (+) and minus (-) like this:
3x6 - 2x5 + 2x3 - 8
But, again, your teacher might have specific instruction for you to follow.
