Vertex form:

y = a(x - h)² + k where (h,k) are the coordinates of the vertex

So all you need to do is replace h and k by the numbers you were given for the vertex

y = 7*(x-(-9))² + 4

so: y = 7(x + 9 )² + 4 is the equation
