In this case, the words provided sort of give away a simple plot: someone who is unable to afford the cost of care, and the verbs "forgive" and "request" certainly point to a kind of pathos/ethos storyline. In order to make the story "pop" start with that basic thread of a poor man asking a doctor to forgive the cost of care, and then add adjectives, or adverbs.
It's much more vivid to write "The kind, old doctor, his brow furrowed and his eyes softened by the years understood the man's plight and decided to waive the cost of care..." instead of "the doctor understood and forgave the cost."
