The "?" - the question mark - is like a period.
It ends the sentence.
To combine questions together into one sentence, you will put them together like a list, and end the list with a question mark.
So instead you would punctuate this like if you wanted to make it once sentence:
What is your name, where do you live, and how old are you?
List items in a sentence are usually separated by commas " , ", and the last item in the list has the word "and" before it.
