Point-slope form is: y - y1 = m(x - x1) where "m" is the slope, and the point the line goes through is (x1, y1). So the equation of the line with a slope of 5/4 that goes through (4, -9) would be y - (-9) = (5/4)(x - 4) or y + 9 = 5/4(x - 4)
