Hello Scharlene,
The correct answer would be #3. Per CPT coding guidelines, the correct code is based on the insertion (venous vs arterial) and if its an initation or subsequent service. It is also important to read the guidelines listed in the CPT book that provides instructions on what must be included and documented to support the use of these codes.
Hint...your CPT book may also have a ECMO chart that makes it easier to locate the correct codes. :<)
