Hello, and thank you so much for your question/specs! While I can't provide a layout in an answer I can give you my top tips I follow when I see project specs like these.
TL; DR: Hierarchy, hierarchy, hierarchy!
Do take the time to figure out which information in the response is most important, followed by the next thing, using basic design principles to distinguish between elements and tell your story. In my line of work, we tend to think about reaching a customer in terms of attention, the most valuable resource our audience has.
In general you have 3 seconds to gain an emotional buy-in from your message. This sometimes comes as a graphic or a succinct, and catchy headline - something your audience can digest in seconds and agree to commit more attention to your message. At that point, you have 30 seconds, or enough time for an elevator pitch, an executive summary of what you want to say. This could be dates listed, a short reminder, or a notification to take the next step and provide some information. For simple layouts, my third tier. or 3 minute buy-in, is the longer copy - or detailed directions to complete the specific steps of my request: Asking for a survey, uploading a detailed report, handling appointment rescheduling, etc. Anything that requires more than a moment or two of concentrated effort.
