I believe you ask for a sentence or situation like this "Die Sache wäre entschieden gewesen, wenn der Chef hier gewesen wäre."
In part one we have three verbs because it is a Konjunktiv, Passive and also a Present Perfect sentence, "wäre" indicating the Konjunktiv, "entschieden" for the Passive, "gewesen" for the Present Perfect,
Here is the same part of the sentence in just the Passive Voice:
"Die Sache ist entschieden"
Now this sentence in the Present Perfect:
"Die Sache ist entschieden gewesen" (alternatively, in most cases, one could say "Die Sache war entschieden.")
BECAUSE (in my example the boss was not present) there is a reason why it is a statement about an unreal situation and we use the Konjunktiv, therefore it needs to be
"Die Sache wäre entschieden gewesen..."
