I think what your asking is: Put the given values in the point slope formula?
If so...
Use the formula:
y-y1=m(x-x1)
Then plug in your values. Use the point (0,4) as x1,y1 and the slope (9/2) as x.
y-4=(9/2)(x-0)
Now simplify.
y-4=(9/2)(x-0)
y=(9/2)x+4
y=(9/2)x+4
Solution: y=(9/2)x+4
